Reworked Exhaustion, Exposure, ambushes, healing

Now, instead of reducing Inventory, Exposure and Exhaustion add items of Cold and Tired to your load. Hopefully easier to track, has the same effect of penalizing your rolls (and making Dangerous Actions more likely to be fatal).
Healing also modified so that eating, drinking, and sleeping in shelter restores Health and removes Cold and Tired.
Changed ambushes to give advantage / disadvantage in first action after ambush.
Also tweaked description of Shooting but didn't really change mechanics.
